Report a motorcycle training incident to DVSA
You must report any incident that happens during motorcycle training to the Driver and Vehicle Standards Agency (DVSA).
Applies to England, Scotland and Wales
Approved training bodies (ATB) or your instructors must use this online form to report any incidents that happen during:
- compulsory basic training
- direct access scheme training (on and off road)
- training at a DVSA multi-purpose test centre
You should call DVSA straight away to report a serious incident (any incident that involves injuries such as broken bones, or an injury that leads to hospitalisation)
Report a motorcycle training incident
You must report the incident within 48 hours of it happening.
Your report must be an accurate account of the incident as it may be used for legal purposes.
Before you start
You need:
- certificate number of the instructor involved
- your approved training body (ATB) number
- the registration numbers (number plates) of the vehicles involved
- name and contact number of any police officers (if the police were involved)
- crime number (if the police were involved)

What happens next
DVSA may contact you to further investigate the incident.
DVSA may share the report with:
- the Health and Safety Executive
- solicitors in response to a civil or injury claim
- the police where it’s part of an investigation and it’s an on-road incident
