This form should be completed for each new case (including cases diagnosed at post mortem) occurring in England, Wales and Northern Ireland of culture confirmed disease due to Mycobacterium bovis, isolated at a Reference Centre (Regional Centre for Mycobacteriology or at the National mycobacterium reference service, NMRS-South).
