Following the release of the National Information Board’s vision document ‘Personalised Health and Care 2020’ (PHC2020) in November 2014, these final roadmaps describe in more detail how the proposals made in PHC2020 will be taken forward over the next 5 years.

Initially published in draft form in June 2015, the roadmaps have been taken through a process of engagement with key stakeholder groups to challenge assumptions, ratify direction, amend and refine the content.
