Details

This request asked for the number of medication errors reported to NRLS with the degree of harm recorded as death and where the incident happened (ie acute, mental health etc). It also asked for a summary of each incident showing the name and quantity of drug that should have been received and quality of drug they did receive, and the medication incident category. The request covered the period 2014 to 2016.

NHS Improvement held the information requested and decided to release it.
