Details

Public Health England (PHE) collects information about users of alcohol and drug misuse services. This includes the outcomes of treatment.

These forms should be used by community treatment providers. The adult combined review form should be used by adult treatment services. The young persons combined review form should be used by young persons treatment services. These forms combine: treatment outcomes profile (TOP) young persons outcome record (YPOR), sub intervention review (SIR) and client information review (CIR).

Keyworkers can use the TOP and YPOR elements as a clinical tool to support the treatment planning process. It can help structure conversations with clients and set treatment goals.

Each downloadable form includes guidance on how to complete them. Full guidance is available in the NDTMS business definitions for community services.

The editable forms are designed to be completed electronically. The standard forms are designed to be printed and completed as hard copies.
