Details

Public Health England (PHE) collects information about users of alcohol and drug misuse services. This includes the outcomes of treatment.

These forms should be used in the secure setting. Keyworkers can use the treatment outcomes profile as a clinical tool to support the treatment planning process. It can help structure conversations with clients and set treatment goals.

PHE collects secure setting treatment outcome information through the treatment outcomes profile (TOP) for adults and the young people’s outcomes record (YPOR) for young people.

All downloadable forms include guidance on how to complete them. Full guidance is available in the National Drug Treatment Monitoring System (NDTMS) business definitions for secure settings.

The editable forms are designed to be completed electronically. The printable forms are designed to be printed and completed as hard copies.
