Newborn hearing screening: equipment log sheets
Use these log sheets to record and review equipment use in local Newborn Hearing Screening Programmes (NHSP).

Details
Local hearing screening programmes must keep these log sheets with equipment at all times.
Complete checks:
-
every day that the equipment is used for screening
-
under the correct Quality Assurance (QA) number in the equipment
Review the stored results on the equipment to ensure they have been stored under the correct QA number.
On the log sheet, record:

the outcome of the checks by circling Y or N
-
anything significant about the equipment such as probe lead changes, faults or returns to the manufacturer
Equipment that fails a test must not be used for screening until the problem is resolved.
