Details

Secretary of State for Health and Social Care Jeremy Hunt announced in November 2017 that the HSIB would investigate:

• all cases of early neonatal deaths, term intrapartum stillbirths and cases of severe brain injury in babies (Each Baby Counts cases) in England
• all cases of maternal deaths in England

These directions came into effect on 30 April 2018 and cover HSIB’s additional functions and duties in carrying out maternity investigations. They should be read alongside the National Health Service Trust Development Authority (Healthcare Safety Investigation Branch) Directions 2016.
