Details

This form should be completed by a pet owner or authorised person who is travelling with more than 5 pet dogs, cats or ferrets to attend a competition, show, sporting event or training, and who wishes to travel under the rules of the Pet Travel Scheme.

The document is a declaration that the pet animals are not being moved for commercial purposes and that you do not intend to sell, or transfer ownership of, the animals.

You don’t need to send this declaration anywhere. However, you need to complete it and keep it with you when you move with your pet.
