Use this form to apply for an accessibility certificate test for a bus or coach.

Send the form online

  1. Download and fill in the application form.

  2. Save the application form on your computer.

  3. Send the application form to the Driver and Vehicle Standards Agency (DVSA) using the service to apply for a certificate for a coach or bus.

  4. Pay the application fee by credit or debit card (DVSA will tell you how to do this after you’ve applied), or by cheque or using a pre-funded account.
