Section 1 (Reference)
This is optional, we will use it in correspondence if included.
Section 2 (Design numbers)
Enter the design application number, or registration number(s) to process this form.
Section 3 (Name of applicant or owner)
Enter the name of the applicant or owner as it appears on the registration certificate, or as it was entered on the application form if it is not yet registered.
Section 4 (Transaction type)
Select the box for Recording a change of ownership or for Recording or cancelling a Licence or Security.
If you are recording a change of ownership complete sections 5 and 6 then go to section 9.
If you are recording or cancelling a licence or security complete sections 7 and 8.
Section 5 (Change of ownership)
Enter the full name, address, postcode and email address of the new owner.
Section 6 (Stamp Duty)
Select the box No stamp duty is payable or Stamp duty has been paid.

Section 7 (Record or cancel a licence or security)
Complete this if you are recording or cancelling a licence or security. Select what it is you are recording (R) or cancelling (C)
Section 8 (Name and address)
Enter the full name, address, postcode and email address of the licensee, or person granted a security against the design.
Section 9 (Date of transfer, licence or security)
Enter the exact day, month and year on which the transfer of ownership (assignment), licence or security agreement took place. This must not be a date in the future.
Section 10 (Name, address and telephone number)
Enter the name, address, telephone number and email address of the person to send confirmation to, if different to that in Section 3.
Section 11 (Authorisation)
Sign the form and enter the date the form is filed.
This can be typed or handwritten.
Section 12 (Contact)
Enter the name, daytime telephone number and email address of the person to contact in case of a query.
Section 13 (Attachments)
Enter the number of attachments included with this form.
