Register a Place of Destination form

Details

If you are involved in importing forestry plants for planting, please refer to the Place of destination checks on high-priority plants and products from the EU from 1 January 2021 page.

Use this form to register a Place of Destination where physical inspections of regulated EU material (and third-country regulated material transiting through the EU) will take place instead of at the border. This will apply to regulated material that would have needed a Plant Passport prior to 1 January 2021 (but after 1 January 2021 requires a Phytosanitary Certificate).

This form can be used for submitting the details of up to 5 individual Places of Destination attributed to a single Professional Operator.

Before completing and submitting this application form, please ensure you are registered as a Professional Operator with the Forestry Commission (you will need your Professional Operator Registration Number to continue).

How to register as a Place of Destination

Please complete all relevant or required sections in this form and click ‘submit’. You will receive a confirmation of submission. It is crucial that your email address is entered correctly. Your application will be reviewed by Forestry Commission Plant Health Forestry. Once all necessary criteria are met, your Place of Destination will be approved by the Forestry Commission and you will be advised of the approval via email.

Do not use this form for:

  • registering as a Professional Operator
  • notifying the competent authority of imports of regulated wood, wood products and bark, or
  • to apply for an Export Phytosanitary Certificate for wood or wood products.

How to change your contact details

If you wish to change any details after submission, please email Forestry Commission Plant Health Forestry directly at plant.health@forestrycommission.gov.uk or call the Plant Health Admin team on 0300 067 5155.
