Staphylococcus capitis minimum data collection form

Use this form for capturing core epidemiological information on babies for whom isolates are referred to the reference laboratory.

Additional isolates are no longer needed for the purpose of describing the epidemiology of S. capitis NRCS-A in England.

We do however still encourage labs to send any S. capitis isolate that is suspected to belong to a cluster or outbreak and meets the case definition from the latest briefing note (Briefing Note 2021/078, issued 15 November 2021). These isolates can be sent to the Staphylococcus reference laboratory under code ‘NRCS-A’ using the relevant H3 or H4 forms.
