Use this form if you’re a DVSA earned recognition operator and you:

• need to tell DVSA about any changes to your business that may affect your status
• want to stop one of the extra modules

Fill in the form

1. Download and fill in the application form.

2. Click the Submit button at the bottom of the form to send it to the Driver and Vehicle Standards Agency (DVSA).

Send your application form by email

Save the application form on your computer and send it as an attachment to dvsaer@dvsa.gov.uk.
