This review is in response to a Parliamentary and Health Service Ombudsman (PHSO) finding in July 2014 of maladministration by the Home Office between November 2010 and December 2012. The PHSO report was instigated by a complaint by a member of the public Mrs A who, along with her family, had suffered serious harm resulting from the crimes of a Canadian national who had obtained a visa to enter the UK.
