This report presents data against each of the 15 screening standards for screening activity related to abdominal aortic aneurysm (AAA) screening.
The data source used for this report is AAA SMaRT (screening management and referral tracking).
The NHS AAA screening programme (NAAASP) aims to reduce aneurysm-related mortality through early detection, appropriate monitoring and treatment. NAAASP invites men for ultrasound screening during the year (1 April to 31 March) in which they turn 65. Men over 65, who have not previously been screened can self-refer. The programme offers annual or quarterly surveillance for all men who on initial scan are found to have a small or medium aneurysm respectively. Men found to have a large aneurysm are referred into the care of specialised vascular providers for diagnosis and treatment.
National AAA standards provide a defined set of measures that providers must meet to make sure services are safe and effective. NAAASP published a set of revised standards in April 2020.
This report is an official statistic. Official statistics are an essential public asset. They provide a window on society, the economy, and on the work and performance of government. They are fundamental to the judgements and decisions made by the public, by government, and by an enormous range of other organisations.
During March 2020, all 38 local NHS AAA screening providers stopped inviting men temporarily due to the impact of the COVID-19 pandemic. National restoration guidance was developed to make sure that men could attend their appointments safely. Local services restarted screening between June and August 2020. Most local services made sustained progress between April 2021 and March 2022. However, the pandemic continued to impact on the attainment of standards in some providers.
1. AAA Standards
There are national AAA standards included in this report, which are:
- AAA-S01: completeness of offer (initial screen)
- AAA-S02: completeness of offer (annual surveillance scan)
- AAA-S03: completeness of offer (quarterly surveillance scan)
- AAA-S04: coverage (initial screen)
- AAA-S05: coverage (annual surveillance scan)
- AAA-S06: coverage (quarterly surveillance scan)
- AAA-S07: coverage of the initial screen in men living in deciles 1 to 3
- AAA-S08: uptake (initial screen)
- AAA-S09: uptake (annual surveillance scan)
- AAA-S10: uptake (quarterly surveillance scan)
- AAA-S11: non-visualised screens
- AAA-S12: internal quality assurance
- AAA-S13: nurse assessments
- AAA-S14: men seen at specialist assessments within 2 weeks
- AAA-S15: men operated on within 8 weeks
Standards data are published annually. Standard AAA-S04, AAA-S05 and AAA-S06 are also key performance indicators (KPIs), which are published quarterly.
Two thresholds, the acceptable threshold and achievable threshold are specified for each standard.
1.1 Provider table

2. AAA-S01: completeness of offer (initial screen)
There were 309,439 men aged 65 eligible for AAA screening between 1 April 2021 and 31 March 2022; 88.8% of those eligible men were offered an initial screen within the screening year plus 2 months. The reasons for not being invited include:
- offered after 31 May 2022 (mainly due to some local services still recovering from the impact of the COVID-19 pandemic)

- Leicester AAA Screening Cohort and Coventry and Warwick AAA Screening Cohort had the highest outcome (100.0%)
- Cheshire and Merseyside AAA Screening Cohort had the lowest outcome (43.7%)
- 23 out of 38 screening providers met the acceptable threshold of ≥ 95.0%
- 9 out of 38 screening providers also reached the achievable threshold of ≥ 99.9%

The offer of the initial screen to men in each cohort year was stable between 2013/14 (see previous years reports for data prior to 2017/18) and 2019/20. The performance in 2020/21 dropped sharply due to the impact of the pandemic. For 2021/22, performance has increased, but still below the pre-pandemic level and below the both the achievable and acceptable thresholds. 33 of the 38 providers saw an increase in performance from 2020/21 to 2021/22, with the largest increase seen in Black Country AAA Screening Cohort, from 52.2% to 95.8% in 2021/22.
3. AAA-S02: completeness of offer (annual surveillance scan)
Between 1 April 2021 and 31 March 2022 there were 11,691 annual surveillance appointments due and 11,430 had an offer within 6 weeks of their due date (97.8%).

Nationally the proportion of annual surveillance appointments due where an appointment was offered 6 weeks either side of the due date increased by 23 percentage points from 2020/21 to 2021/22, at 97.8%, meeting the acceptable threshold. All 38 providers saw an improvement in performance between 2020/21 and 2021/22, with the largest improvement seen in South London AAA Screening Cohort, increasing from 46.6% to 94.0% in 2021/22, however they did not meet the acceptable threshold.
4. AAA-S03: completeness of offer (quarterly surveillance scan)
Between 1 April 2021 and 31 March 2022 there were 10,999 quarterly surveillance appointments due and 10,845 had an offer within 4 weeks of their due date (98.6%).

- South London AAA Screening Cohort had the lowest outcome (93.3%)
- 37 out of 38 screening providers met the acceptable threshold of ≥ 95.0%
- 19 out of 38 screening providers also reached the achievable threshold of ≥ 99.0%
The following provider(s) did not meet the acceptable threshold: - South London AAA Screening Cohort
Nationally the proportion of quarterly surveillance appointments due where an appointment was offered 4 weeks either side of the due date increased by 25.6 percentage points from 2020/21 to 2021/22, at 98.6%, meeting the acceptable threshold. 37 of the 38 providers saw an improvement in performance between 2020/21 and 2021/22, with the largest improvement seen in West Surrey AAA Screening Cohort, increasing from 63.8% to 98.9% in 2021/22. Cheshire and Merseyside AAA Screening Cohort saw a decrease in performance, however the acceptable standard was still achieved in 2021/22. Local screening services have worked hard to improve overall performance against this standard and make sure that the men most at risk of a AAA adverse event are screened within national timescales.
5. AAA-S04: coverage (initial screen)
Of the 309,439 men aged 65 eligible for AAA screening between 1 April 2021 and 31 March 2022; 70.2% of those eligible men were conclusively tested within the screening year plus 2 months.

The percentage of eligible cohort men conclusively tested declined in 2019/20, as the impact of the start of the pandemic took hold in quarter 4. In 2020/21, there was a larger decrease seen with full the effect of the pandemic. For 2021/22, performance has increased, but still below the pre-pandemic level and below the both the achievable and acceptable thresholds.
37 of the 38 providers saw an increase in performance from 2020/21 to 2021/22, with the largest increase seen in North and South Staffordshire AAA Screening Cohort, from 49.6% to 83.9% in 2021/22. Only Hertfordshire AAA Screening Cohort saw a decrease in performance in 2021/22, compared with 2020/21.
6. AAA-S05: coverage (annual surveillance scan)
Of the 11,691 annual surveillance appointments due between 1 April 2021 to 31 March 2022, 91.0% had a conclusive test within 6 weeks of the due date.

Local screening services have worked hard to improve overall performance against this standard and make sure that the men most at risk of a AAA adverse event are screened within national timescales.
8. AAA-S07: coverage of the initial screen in men living in deciles 1 to 3
Nationally for 2021/22, 62.7% of men in the eligible cohort who were tested and lived in a lower super output area (LSOA) classed as decile 1 to 3 in the English indices of deprivation (IoD) 2019. This contrasts with 70.2% for the overall cohort.

Men living in more deprived areas are less likely to attend for screening but are more likely to have an aneurysm. It is important for providers to engage with men living in more deprived areas to make sure they can make a personal informed choice.
This standard is the second year of being reported and should be reviewed alongside standards 1 and 4. It was first introduced in April 2020, however, performance during 2020/2021 was impacted by the COVID-19 pandemic.
For 2021/22, performance has increased on 2020/21, however it is below the both the achievable and acceptable thresholds. 34 of the 38 providers saw an increase in performance from 2020/21 to 2021/22, with the largest increase seen in North and South Staffordshire AAA Screening Cohort, from 38.1% to 75.9% in 2021/22. This aligns with the large increase in performance this provider saw in standard 4. 4 providers saw a decrease in performance in 2021/22, compared with 2020/21, with largest being a decrease of 4.5 percentage points to 43.6% at Bedfordshire Luton and Milton Keynes AAA Screening Cohort.
9. AAA-S08: uptake (initial screen)
There were 274,816 men offered AAA screening between 1 April 2021 and 31 March 2022; 79.0% of those men offered an initial screen were conclusively tested within the screening year plus 2 months.

The percentage of men offered screening who were conclusively tested has remained relatively stable but saw a small decrease from 2018/19 to 2019/20. For 2021/22, performance has increased slightly on 2020/21, and continues to meet the acceptable threshold.
26 of the 38 providers saw an increase in performance from 2020/21 to 2021/22, with the largest increase seen in South London AAA Screening Cohort, from 61.7% to 73.0% in 2021/22, however this is still below the acceptable threshold.

Of the 11,430 annual surveillance appointments that were offered within 6 weeks of the due date, 93.1% had a conclusive test within the 6 weeks between 1 April 2021 to 31 March 2022.

The percentage of non-visualised screens has been decreasing across England over the series. For 2021/22, performance came down 0.1 percentage point from 2020/21, and met the achievable threshold for the first time. 21 of the 38 providers improved their performance from 2020/21 to 2021/22, with the largest decrease seen in Five Rivers AAA Screening Cohort, from 3.6% to 0.8% in 2021/22, meeting the achievable threshold.
13. AAA-S12: internal quality assurance
Of the 2,816 men with a small or medium size aneurysm (aorta ≥ 3.0 to <5.5cm), 86.0% had their images reviewed within 21 days of the scan taking place.

This was the only standard in 2021/22 that saw a reduction in performance, when compared with the previous year.
18 of the 38 providers saw an increase in performance from 2020/21 to 2021/22, with the largest increase seen in South London AAA Screening Cohort, from 24.6% to 51.4% in 2021/22, however this was below the acceptable threshold. North Yorkshire and Humber AAA Screening Cohort saw the largest reduction, down to 26.4%, from 82.0% in 2020/21.
15. AAA-S14: men seen at specialist assessments within 2 weeks
Of the 972 men with an aorta ≥ 5.5cm, 900 (92.6%) were seen by a vascular specialist within 2 weeks of their last conclusive scan.

The percentage of men operated on within 8 weeks decreased nationally from 2018/19 to 2020/21. For 2021/22, performance has improved 2.7 percentage point from 2020/21, but it is still considerably less than the acceptable threshold.
26 of the 38 providers improved their performance from 2020/21 to 2021/22, with the largest increase seen in West Yorkshire AAA Screening Cohort, going from 0% to 43.8% in 2021/22, however this was still short of the acceptable threshold. The largest decrease was in Thames Valley AAA Screening Cohort, which fell to 14.3% (from 50% in 2020/21).
17. Outcomes
There were 39 AAA ruptures during the screening year, 27 of which were fatal. 16 of the deaths were in men on surveillance (3.0% of the deaths in surveillance) and 11 had been referred for surgery. For the men referred to surgery, 4 had been turned down for surgery or had declined the operation and 7 died while waiting for surgery. Of the 7, 4 died after waiting more than 8 weeks for surgery. In the 2020/21, 7 men died while waiting for surgery, 5 of which died after waiting more than 8 weeks from referral.
