This report presents data against each of the 15 screening standards for screening activity related to abdominal aortic aneurysm (AAA) screening.
The data source used for this report is AAA SMaRT (screening management and referral tracking).
The NHS AAA screening programme aims to reduce aneurysm-related mortality through early detection, appropriate monitoring and treatment. The NHS AAA screening programme invites men for ultrasound screening during the year (1 April to 31 March) in which they turn 65. Men over 65, who have not previously been screened can self-refer. The programme offers annual or quarterly surveillance for all men who on initial scan are found to have a small or medium aneurysm respectively. Men found to have a large aneurysm are referred into the care of specialised vascular providers for diagnosis and treatment.
National AAA standards provide a defined set of measures that providers must meet to make sure services are safe and effective. The NHS AAA screening programme published a set of revised standards in April 2020.
This report is an official statistic. Official statistics are an essential public asset. They provide a window on society, the economy, and on the work and performance of government. They are fundamental to the judgements and decisions made by the public, by government, and by an enormous range of other organisations.
1. AAA Standards
There are 15 national AAA standards included in this report, which are:
- AAA-S01: completeness of offer (initial screen)
- AAA-S02: completeness of offer (annual surveillance scan)
- AAA-S03: completeness of offer (quarterly surveillance scan)
- AAA-S04: coverage (initial screen)
- AAA-S05: coverage (annual surveillance scan)
- AAA-S06: coverage (quarterly surveillance scan)
- AAA-S07: coverage of the initial screen in men living in deciles 1 to 3
- AAA-S08: uptake (initial screen)
- AAA-S09: uptake (annual surveillance scan)
- AAA-S10: uptake (quarterly surveillance scan)
- AAA-S11: non-visualised screens
- AAA-S12: internal quality assurance
- AAA-S13: nurse assessments
- AAA-S14: men seen at specialist assessments within 2 weeks
- AAA-S15: men operated on within 8 weeks
Standards data are published annually. Standard AAA-S04, AAA-S05 and AAA-S06 are also key performance indicators (KPIs), which are published quarterly.
Two thresholds, the acceptable threshold and achievable threshold are specified for each standard.
1.1 Provider table

Data are presented by financial year (1 April 2022 to 31 March 2023) unless stated otherwise.
2. AAA-S01: completeness of offer (initial screen)
There were 324,555 men aged 65 eligible for AAA screening between 1 April 2022 and 31 March 2023; 97.0% of those eligible men were offered an initial screen within the screening year plus 2 months. The reasons for not being invited include:
- offered after 31 May 2023 (mainly due to some local services still recovering from the impact of the COVID-19 pandemic)

The offer of the initial screen to men in each cohort year was stable between 2013/14 (see previous years reports for data prior to 2017/18) and 2019/20. Following the sharp drop in 2020/21 performance due to the impact of the pandemic, performance increased in 2021/22, and has increased further in 2022/23, meeting the acceptable threshold, and has almost returned to the pre-pandemic levels.
36 of the 38 providers saw an increase in performance from 2021/22 to 2022/23, with the largest increase seen in Manchester AAA Screening Cohort, from 69.9% to 99.9% in 2022/23.
3. AAA-S02: completeness of offer (annual surveillance scan)
Between 1 April 2022 and 31 March 2023 there were 12,344 annual surveillance appointments due and 12,056 had an offer within 6 weeks of their due date (97.7%).

- West Yorkshire AAA Screening Cohort Nationally the proportion of annual surveillance appointments due where an appointment was offered 6 weeks either side of the due date decreased by 0.1 percentage points from 2021/22 to 2022/23, at 97.7%, however still meeting the acceptable threshold.
22 providers saw an improvement in performance between 2021/22 and 2022/23, with the largest improvement seen in Lancashire and South Cumbria AAA Screening Cohort, increasing from 94.0% to 98.5% in 2022/23. West Yorkshire AAA Screening Cohort saw the largest year on year decrease, with performance decreasing from 97.9% to 72.8% in 2022/23.
4. AAA-S03: completeness of offer (quarterly surveillance scan)
Between 1 April 2022 and 31 March 2023 there were 11,423 quarterly surveillance appointments due and 11,248 had an offer within 4 weeks of their due date (98.5%).

- 15 out of 38 screening providers also reached the achievable threshold of ≥ 99.0%
Nationally the proportion of quarterly surveillance appointments due where an appointment was offered 4 weeks either side of the due date decreased by 0.1 percentage points from 2021/22 to 2022/23, at 98.5%, however still meeting the acceptable threshold.
16 of the 38 providers saw an improvement in performance between 2021/21 and 2022/23, with the largest improvement seen in South London AAA Screening Cohort, increasing from 93.3% to 97.4% in 2022/23. Leicester AAA Screening Cohort saw the largest decrease in performance, falling 3 percentage points to 96.6%, however they still achieved the acceptable standard.
5. AAA-S04: coverage (initial screen)
Of the 324,555 men aged 65 eligible for AAA screening between 1 April 2022 and 31 March 2023; 78.4% of those eligible men were conclusively tested within the screening year plus 2 months.

Following the decrease in performance in 2020/21, due to the impact of the pandemic, performance increased in 2021/22, and has again increased further in 2022/23, meeting the acceptable threshold and edging towards to pre-pandemic levels again.
29 of the 38 providers saw an increase in performance from 2021/22 to 2022/23, with the largest increase seen in North East and North Cumbria AAA Screening Cohort, from 31.1% to 78.5% in 2022/23. Peninsula AAA Screening Cohort saw the largest decrease in performance from 81.4% in 2021/22, to 66.7% in 2022/23.
6. AAA-S05: coverage (annual surveillance scan)
Of the 12,344 annual surveillance appointments due between 1 April 2022 to 31 March 2023, 90.9% had a conclusive test within 6 weeks of the due date.

Men living in more deprived areas are less likely to attend for screening but are more likely to have an aneurysm. It is important for providers to engage with men living in more deprived areas to make sure they can make a personal informed choice.
This standard is the third year of being reported and should be reviewed alongside standard 4. It was first introduced in April 2020, however performance during 2020/2021 was impacted by the COVID-19 pandemic. Performance has increased year on year, however it is below both the achievable and acceptable thresholds.
28 of the 38 providers saw an increase in performance from 2021/22 to 2022/23, with the largest increases seen in Bedfordshire Luton and Milton Keynes AAA Screening Cohort, from 43.6% to 64.8% and Manchester AAA Screening Cohort, from 45.6% to 66.7%, in 2022/23. The largest decrease was for Sussex AAA Screening Cohort of 14.6 percentage points to 59.6%.
9. AAA-S08: uptake (initial screen)
There were 314,865 men offered AAA screening between 1 April 2022 and 31 March 2023; 80.8% of those men offered an initial screen were conclusively tested within the screening year plus 2 months.

The percentage of non-visualised screens is stable across England over the series. For 2022/23, performance remained at 1.0%, and met the achievable threshold.
22 of the 38 providers improved their performance from 2021/22 to 2022/23, with the largest decrease seen in North and South Staffordshire AAA Screening Cohort, from 1.3% to 0.2%, meeting the achievable threshold. The largest increase was seen in Lincolnshire AAA Screening Cohort, from 0.7% to 2.3%

Of the 2,628 men with a small or medium size aneurysm (aorta ≥ 3.0 to < 5.5cm), 84.9% had their svg images for report reviewed within 21 days of the scan taking place.

The largest decrease was seen in Black Country AAA Screening Cohort, from 92.9% to 44.4%.
14. AAA-S13: nurse assessments
Of the 2,927 men who had a small or medium aneurysm detected (aorta ≥ 3.0 to < 5.5cm) at initial screen and the number of men who had a medium aneurysm detected (aorta ≥ 4.5 to < 5.5cm) at the annual surveillance scan, 68.0% had a nurse assessment within 12 weeks of their conclusive scan.

15. AAA-S14: men seen at specialist assessments within 2 weeks
Of the 990 men with an aorta ≥ 5.5cm or men with an aorta ≥ 4.0cm that has grown ≥ 1cm in one year, 900 (93.1%) were seen by a vascular specialist within 2 weeks of their last conclusive scan.
N.B. men who had an aorta greater than or equal to 4.0cm that has grown 1cm or more in one year were included for first time in 2022/23 data.

The percentage of men operated on within 8 weeks is gradually starting to increase nationally. For 2022/23, performance has improved 0.5 percentage point from 2021/22, but it is still considerably less than the acceptable threshold.
17 of the 38 providers improved their performance from 2021/22 to 2022/23, with the largest increase seen in Gloucester AAA Screening Cohort, increasing from 8.7% to 57.9% in 2022/23, however this was still short of the acceptable threshold. The largest decrease was in Peninsula AAA Screening Cohort, which fell to 28.6% (from 68.8% in 2021/22).
17. Outcomes
There were 42 AAA ruptures during the screening year, 29 of which were fatal. 16 of the deaths were in men on surveillance (2.9% of the deaths in surveillance) and 13 had been referred for surgery. For the men referred to surgery, 5 had been turned down or had declined the operation and 8 died while waiting for surgery. Of the 8, 5 died after waiting more than 8 weeks for surgery. In 2021/22, 7 men died while waiting for surgery, 4 of which died after waiting more than 8 weeks from their referral.
