National flu report summary: 22 November 2018 (week 47)

Main points: data up to week 46, 2018

Data up to week 46 (ending 18 November 2018) shows:

• influenza activity remains low with sporadic cases of influenza detected in the community and all indicators below baseline threshold levels.
• the impact of flu on healthcare services is below baseline threshold levels for hospitalisations and ICU and HDU admissions
• RSV activity continues to increase with impact particularly in young children

Surveillance scheme summaries

Community

Twenty-two new acute respiratory outbreaks have been reported in the past 7 days. Fifteen outbreaks were reported from care homes where 1 tested positive for influenza A(not subtyped), 1 was positive for RSV, 5 were positive for rhinovirus,1 tested positive for human metapneumovirus (hMPV) and another was positive for enterovirus. Three outbreaks were reported from hospitals where 1 tested positive for influenza A(H1N1)pdm09, another tested positive for influenza A(H3) tested positive and the third was positive for RSV. One outbreak was reported from a school which tested positive for RSV. The remaining 3 outbreaks were from the Other settings category, where 2 tested positive for influenza A(not subtyped)

Primary care

Data from primary care surveillance shows:

• the rate of influenza-like illness (ILI) was below baseline threshold levels
• the overall weekly ILI GP consultation rate was 5.2 per 100,000 registered population in participating GP practices for England, similar to 5.3 per 100,000 in week 45
• in the devolved administrations, ILI rates were also below baseline threshold levels

Secondary care

Data from secondary care surveillance shows:

• hospitalisation rate observed was below baseline threshold levels, with a rate of 0.20 per 100,000 trust catchment population for England (21 NHS Trusts), this is similar to 0.19 per 100,000 in week 45
• ICU and HDU admission rate observed was below baseline threshold levels, with a rate of 0.02 per 100,000 trust catchment population for England (136 out of 143 NHS Trusts), this is the same as 0.02 per 100,000 in week 45
• there was one new influenza admissions reported from the 6 severe respiratory failure centres in the UK

All-cause mortality

Data from all-cause mortality surveillance shows:

• no statistically significant excess all-cause mortality by week of death was seen overall and by age group in England
• in the devolved administrations, no statistically significant excess all-cause mortality for all ages was observed in Scotland, Northern Ireland and Wales

Microbiological surveillance

Data from microbiological surveillance shows:

• in primary care 1 sample tested positive for influenza (1 influenza A(H1N1)pdm09) with a positivity of 2.4% through the UK GP sentinel schemes
• in secondary care influenza percent positivity was 2.5%, below baseline threshold levels, this is similar to 2.6% in week 45
• a total of 46 detections were recorded through the DataMart scheme (16 influenza A(H1N1)pdm09, 6 influenza A(H3), 19 influenza A(unknown subtype) and 5 influenza B)
• RSV positivity continues to increase at 17.9% with the highest positivity amongst the less than 5 year olds stabilising at 42.0%

Vaccination

Data from the GP patient, healthcare worker and primary school-age children flu vaccine uptake programmes shows:

• up to week 46 2018, in 94.8% of GP practices in England, the provisional proportion of people who had received the 2018/19 influenza vaccine in targeted groups was 36.6% in under 65 years in a clinical risk group, 37.8% in pregnant women and 57.7% in those aged 65 and older
• in 95.1% of GP practices reporting for the childhood collection the provisional proportion vaccinated was: 31.6% in 2 year olds and 32.4% in 3 year olds
• provisional data from the first monthly collection of influenza vaccine uptake by frontline healthcare workers show 46.3% were vaccinated by 31 October 2018, compared to 46.0% vaccinated in the previous season by 31 October 2017
• provisional data from the first monthly collection of influenza vaccine uptake for children of school years reception to year 5 shows 20.5% in school year reception age, 20.7% in school year 1 age, 19.9% in school year 2 age, 19.7% in school year 3 age, 18.9% in school year 4 age and 18.2% in school year 5 age were vaccinated by 31 October 2018

International situation

• in the temperate zone of the Northern hemisphere, influenza activity remained at inter-seasonal levels. Increased influenza was reported in some countries of Southern and South East Asia
• in the temperate zone of the Southern hemisphere, influenza activity returned to nearly inter-seasonal levels
• worldwide, seasonal influenza subtype A viruses accounted for the majority of detections
