Add a setting to your application to register a daycare (EYS)
You must tell Ofsted about the premises you want to include in your application to register a nursery or other daycare organisation.

There is separate guidance if you are adding a setting to a provider that is already registered with Ofsted.
As part of your application to register a nursery or other daycare you must include an application for each setting you want to provide care at.
Ofsted must approve each nursery or other daycare premises. It is an offence to provide childcare on non-approved premises.
You will need to pay an approval fee and annual fee for each setting where childcare is provided.
Who should complete this form
You should be the same person who completes the application form to register as a nursery or other daycare.
Information you will need
To complete the form, you will need to know:
- your submission reference number – you will receive this when you complete your application to register as a nursery or other daycare (EYO)
- the details of your proposed childcare setting
- who will manage the daycare and any person to contact
- who will look after the children
You will also need to tell Ofsted:
- when you will be providing the childcare
- any additional relevant information you think we should know in relation to carrying out a visit or inspection
- the register, or registers, you want your daycare to join, or why you don’t need to register
Schools
If you are providing daycare in a school, you will also need:
- the school’s unique reference number
- to know whether you are applying to provide daycare as part of a children’s centre
- the number of children and their ages

You must use the online form to give Ofsted the necessary information on your new setting.
