Use these forms to update or amend information in your claim for redundancy.
You’ll need your:
- redundancy claim reference number (this begins with LN)
- National Insurance number
Contact the insolvency practitioner handling your employer’s insolvency if:
- you are not sure about the information you need to provide
- you’re making changes to any details in your claim
They may also need to change the information they provided to us.
Unpaid wages
We need to know about any wages and other money you were owed by your employer, up to your last official date of working.
Use our online service to amend details about your unpaid wages.
If you cannot use the online service, complete a paper form and return it using our general enquiry form. It may take us longer to process forms received this way.

Holiday pay accrued
We need to know about the number of holiday days you were entitled to. It’s important these details are correct or we cannot pay you.
Use our online service to amend details about your accrued holiday pay.
You’ll need details of your:
- holiday leave year start
- number of days you were entitled to take during your whole leave year
- number of days you took between the start of your leave year and the date you were made redundant
- number of days you carried over from your last leave year allowance

Holiday taken but not paid
We need to know about the holiday days you took but were not paid for. It’s important these details are correct or we cannot pay you.
Use our online service to amend details about your holiday taken but not paid.
You’ll need details of your:
- holiday leave year start
- number of days you were entitled to take during your whole leave year
- dates you took holidays but did not receive pay for

Your employment details
We need to know the exact information found in your employment contract and on your payslips. It’s important these details are correct or we cannot pay you.
Use our online service to amend the following details about your employment:
- your gross weekly pay
- employment start and end date
- the date your notice was given
These are the only details you can update, which we use to calculate your claim.

Your personal details
We need to know the exact information found in your personal documents and on your payslips. It’s important these details are correct or we cannot pay you.
Use our online service to amend the following details about your:
- name
- National Insurance number
- date of birth
- email address
- home address
- mobile number
If you cannot use the online service, complete a paper form and return it using our general enquiry form. It may take us longer to process forms received this way.
Your bank details
Complete the bank details amendment form and return it to us using our general enquiry form.
