Before you start

If applying for a specific licence to freeze germinal product from susceptible animals in a bluetongue zone you will need to download and complete an EXD701 application form (PDF, 562 KB, 3 pages) before you start.

We recommend preparing the following information before you start:

  • the licensee’s address and contact details
  • the address where germinal product would be frozen, or the addresses where germinal product would be moved from and to (as appropriate)
  • county parish holding (CPH) numbers
  • designated premises approval number (if you have one)
  • animal identification marks (if available) such as ear tags, freeze brands, slap marks and tattoos
  • stock vehicle and driver details (if available) – we’ll only ask for these if you’re applying for a licence to move germinal product, rather than freeze it
