Use this form to nominate someone for a Merchant Navy Medal for Meritorious Service.
The Merchant Navy Medal for Meritorious Service is for individuals who have set an outstanding example to others by their devotion to duty or exemplary service.
Who can you nominate for a medal?
You can nominate anyone who is serving, or has served, in the Merchant Navy or fishing fleets of the United Kingdom, Isle of Man, the Channel Islands or the British Overseas Territories.
Your nomination will need to demonstrate why they deserve to be awarded the medal.
Do not tell the person you are nominating about the nomination.
Who can make a nomination?
Anyone can make a nomination. You are not allowed to nominate yourself.
When to make a nomination
You can make a nomination whenever you want. If you want a nomination to be considered for that year’s awards, you will need to submit it by 31 May.
What you will need to complete the form
To complete the nomination form you will need to provide:
- a description of what the person has done to deserve the medal
- letters of support from at least 2 other people who know the nominee professionally
- personal and contact information for the person you are nominating
- employment details for the person you are nominating

Number of medals awarded
We award a maximum of 20 medals each year.
Not everyone who is nominated may receive a medal, and individual nominations may be unsuccessful.
Unsuccessful nominations and re-nominations
If a nomination is unsuccessful, you may re-nominate the same person. It is unlikely the nomination will succeed unless your nomination can demonstrate additional achievements.
We announce the people who have been awarded a medal towards the end of each year.
If you have not heard that your nomination is successful within 2 years, you can assume that it was not successful.
