If the Declaration Category selected in Step 1 of the Step-By-Step Guide (section CDSSG06000) does not permit the desired Requested Procedure then either a different Declaration Category or alternative Requested Procedure must be selected.

• A list of the Requested Procedures available and a short title describing their purpose
