In England, Wales and Scotland, Irish claim forms can be obtained from Irish Tax and Customs website.

In Northern Ireland, a stock of forms should be kept in tax offices for issue to applicants. Further supplies should be obtained as necessary by each tax office from the Controller, Northern Ireland.
