Summary

On 10 March 2012 at 0540 local time, the bulk carrier Seagate collided with the refrigerated-cargo ship Timor Stream 24 nautical miles north of the Dominican Republic.

Safety Issues

  • poor watchkeeping standards driven by complacency
  • the officer in charge of the navigational watch on both vessels failed to keep a proper lookout, did not assess the risk of, or take appropriate action to avoid the collision

Recommendations

The managers of both vessels have taken action designed to prevent similar accidents in the future which address the safety issues identified in our investigation. Accordingly, no recommendations have been issued with our report.
