Deckhand fell down open fish room hatch when transferring empty bins from upper deck to fish room.

A Chief Inspector’s letter was sent to the owner/skipper of the vessel highlighting the need to carry out a full assessment of the risks for any unusual or non routine operation carried out on board, such as transferring fish stowage bins from one deck to another.
