Synopsis

A crewman died on board the fishing vessel Lisa Leanne, whilst it was moored alongside. He had returned to the vessel in the early hours of the morning to sleep onboard, and was smoking a last cigarette in the accommodation space, when he fell asleep or unconscious and his cigarette started a smouldering fire. The crewman died of smoke inhalation without regaining consciousness.

Action taken:

The Chief Inspector has written to the owners of the Lisa Leanne highlighting the benefits of fitting a simple smoke alarm and employing fire resistant materials for upholstery and coverings.
