Synopsis
The cargo vessel Arklow Racer was arriving at the port of Fowey, Cornwall, about to embark a pilot when she entered restricted visibility. Shortly after she grounded at the cliff face one cable south east of Punch Rocks at the entrance to Fowey.
Action taken
The Chief Inspector has written to the vessel’s owners and made three recommendations:

Improve the company ISM audit regime to ensure methods and procedures adopted on passage are in accordance with SOLAS instructions.

Review personnel monitoring and appraisal procedures to ensure employees, specifically masters,are competent to carry out their assigned duties.

Review ISM documentation to improve its effectiveness, particularly pre departure and pre arrival check lists accompanying the passage plan.
