Summary

On 24 August 2001, a crewman sustained an injury during the firing of cannon on sailing vessel Grand Turk while alongside at Portsmouth.

Safety Issues

  • debris from the previous firing was disturbed by the wrong sequence of swabbing and worming, which caused a premature ignition

Recommendations

Recommendations have been made aimed at reducing the risk of similar accidents occurring in the future if implemented.
