Hazard: Regulators containing a potentially defective 1st stage body presents a serious risk of drowning. A variation in the manufacturing process of the 1st stage regulator has been identified, which results in a thinner wall section. An uncontrollable free flow of the 2nd stage could occur if this section ruptured under pressure, causing a catastrophic gas loss that may result in drowning.
Corrective action: Withdrawal of all affected products from the supply chain. Recall of all affected products still in service from end users.

Hazard
Regulators containing a potentially defective 1st stage body presents a serious risk of drowning. A variation in the manufacturing process of the 1st stage regulator has been identified, which results in a thinner wall section. An uncontrollable free flow of the 2nd stage could occur if this section ruptured under pressure, causing a catastrophic gas loss that may result in drowning. The manufacturing defect means the affected regulators may not supply the user with a breathable gaseous mixture and therefore do not meet the relevant essential health and safety requirements of Regulation (EU) 2016/425 on personal protective equipment.
The issue only affects Calypso regulators with specific common part numbers (CPN) and serial numbers provided here.
Corrective action
Withdrawal of all affected products from the supply chain.
