Implementing general practice in Russia: getting beyond the first steps.
Abstract
Most Central and Eastern European countries are replacing the Soviet model of primary care, based on policlinics, with integrated general practice. There is little information on the progress of this transformation. In Russia, implementing general practice has been hampered by an adverse institutional environment, low professional status of the new practitioners, a failure to adapt the health care infrastructure, and weak financing mechanisms. Successful reform will require changes in the legislative and policy framework, better human resource management, mobilisation of resources, and engagement of relevant institutions to overcome resistance to change.
