Use this form if you no longer want to be an attorney under a lasting power of attorney (LPA) – this is called ‘disclaiming’ responsibility.
If you’re an attorney under an enduring power of attorney (EPA) and no longer want to be an attorney, use form EP5.
You can email customerservices@publicguardian.gov.uk to get a document in large print. Include your address and the title of the document you need.
