Use this form if you no longer want to be an attorney under an enduring power of attorney (EPA) – this is called ‘disclaiming’ responsibility.
If you’re an attorney under a lasting power of attorney (LPA) and no longer want to be an attorney, use form LPA005.
You can email customerservices@publicguardian.gov.uk to ask for a document in large print. Include your address and the name of the document you need.
