Details

Download a copy of the form and send it to any new suppliers, or existing suppliers whose details need to be amended.

Suppliers will be asked to give details about their company, such as:

• the company name and payee name, if different
• the company type and description
• annual turnover, VAT number (if relevant) and bank account information

Suppliers must save the completed form as a PDF and return to the person who sent it to them.

The veterinary practice must then send the completed form to ssd.datamanagement@defra.gsi.gov.uk, or
