Board of Inquiry (BOI) into the Squirrel ZJ 247 helicopter crash at Kingscott Valley, Devon on 29 May 2008.
Following publication of this BoI/SI a subsequent independent review of the report, commissioned by the Joint Helicopter Command (JHC), has determined that there were a number of procedural errors in the conduct of the investigation and some inaccuracies in some of its conclusions.
In particular the JHC is not content that adequate care was taken to assess and record properly the opinions of the Squadron Commander, in order to present a properly balanced picture of the events. While the central finding of the BoI/SI remains that the accident involving Squirrel ZJ247 was avoidable, the lack of balance in the investigation has undermined the value of the final report. This fact is regretted by the JHC, which wishes to acknowledge publicly its opinion.
