Vets can use this form to submit samples from cattle for:
- diagnostic testing
- post-mortem examination
Use the ancillary submission form to provide additional samples or information.
You must immediately report any suspected cases of notifiable disease in animals.

If you’re submitting a sample from farmed animals in England or Wales you can use the online Animal Disease Testing Service (ADTS) instead, with free postage, sample tracking and access to results.
Post-mortem examinations
If you need to submit a carcase for a post-mortem examination, you must call the nearest surveillance pathology centre first.
