APHA testing submission forms

Veterinary surgeons can use these forms to submit samples to the Animal and Plant Health Agency (APHA).

Farm animals: disease diagnosis and post-mortem examinations

Wildlife

Export, import and animal breeding control

Tests for export, import and animal breeding control purposes.

APHA Scientific: general commercial testing

Submit samples to APHA Scientific for commercial testing of animals:

  • that are not livestock in England and Wales, for example companion animals
  • outside England and Wales at the time of sampling

OV disease testing forms

Forms for Official Veterinarians (OV) to carry out Brucellosis and Tuberculin testing.

Tuberculosis (TB)

Testing for private vets including applications for private testing and using unapproved TB tests.

Salmonella reporting

Use these forms to:

  • report suspected salmonella
  • submit suspected salmonella isolates to confirm identification

Bacterial identification

Submit samples to identify bacteria through gene sequence tests.

Test To Exclude (TTE) notifiable diseases

Test to exclude notifiable diseases if the disease is not formally suspected, but cannot be excluded from the differential diagnoses.

Animal Health and Welfare Pathway

Submit samples for testing as part of the Animal Health and Welfare Pathway.
