Submit samples from poultry and gamebirds for diagnostic testing for endemic disease or a post-mortem examination by the Animal and Plant Health Agency (APHA).

Vets can use this form to submit samples and carcases from poultry and gamebirds for:
- diagnostic testing of endemic disease
- post-mortem examination
Poultry and gamebirds include:
- chickens
- turkeys
- domestic ducks
- domestic geese
- peafowl
- guinea fowl
- gamebirds, such as pheasant, grouse and partridge
Use the ancillary submission form to provide additional samples or information.
If you suspect bird flu (avian influenza) or Newcastle disease, you must report it immediately by calling:
- 03000 200 301 if you’re in England
- 03003 038 268 if you’re in Wales
- your local Field Services Office if you’re in Scotland
Bird flu is a notifiable disease in poultry and other captive birds. If you do not report it, you’re breaking the law.

Post-mortem examinations
If you need to submit a carcase for a post-mortem examination, you must call the nearest surveillance pathology centre first.
