Details

Vets can use this form for diagnostic tests and post-mortem examinations on:

  • wildlife
  • companion animals
  • zoo animals
  • farmed mammals such as rabbits, deer, alpacas and llamas

Do not use this form for cattle, pigs, sheep, goats, poultry or gamebirds. Use the correct form for:

You must immediately report any suspected cases of notifiable disease in animals.

Post-mortem examinations

If you need to submit a carcase for a post-mortem examination, you must call the nearest surveillance pathology centre first.
