This form is for local authority or care trust use only.
Use this form to tell the Child Benefit Office as soon as it’s clear a child will be looked after by a local authority or care trust for 8 weeks or more.
There’s no need to tell the Child Benefit Office if a child is being looked after for less than 8 weeks.
You’ll need the:
- child’s given names and any other names the child is known by
- date the child started being looked after by the local authority or Care Trust
- contact details of the social worker, locum, duty social worker or clerk attached to the child
