Details

This form is for registered providers, local authorities and unregistered bodies to notify us of a constitutional change.

This may for example include, but is not limited to, a change in your organisation’s ownership, company structure, amalgamations, transfer of engagements, mergers or de-mergers.

You will need to provide full details of the proposed changes, including the relevant details of all affected organisations (where applicable). 

Once completed, this will be submitted to the Provider Onboarding team for review.

After you have submitted the form 

The Provider Onboarding team may contact you to request additional information where required based on the information submitted.
