What this form is for
This form should be used to apply for an interim compensation payment of £100,000 to be paid to the estates of deceased infected beneficiaries, who were registered with a UK Infected Blood Support Scheme or Alliance House Organisations on or before 17 April 2024.

Who should complete this form?
Personal Representatives (in most cases, yourself, or your legal advisor) can use this form to apply for infected blood interim estate compensation.
To apply, the person who has died must have been registered with an existing UK Infected Blood Support Scheme, or former Alliance House Organisation (AHO) scheme, on or before the 17th April 2024.
A personal representative is not eligible to apply if an interim payment has already been made to the infected person prior to their death, to their bereaved partner or to their estate.
Before completing this form
You will need:
- A certified colour copy of grant of Probate / Confirmation / Letters of Administration (In some cases the original grant of probate may be in black and white, a black and white certified copy of this will be accepted)..
- Certified colour copies of your identity documents.
You must have a grant of probate, letters of administration or a grant of confirmation (in Scotland) to apply for this payment. If you already have probate, you do not need to apply for it again. If you’re not sure if probate has already been granted you can:
- search probate records in England in Wales
- search probate records in Northern Ireland
- check for confirmation in Scotland with the local sheriff court
If probate or confirmation has not already been granted, you can usually apply if you’re:
- named as an executor in the will of the person who’s died
- the person’s closest living relative if there is no will
There’s a different process for applying for probate in order to claim an interim payment. You should not apply using the usual process. Read our guidance applying for probate when claiming this interim payment.

If your interim payment application is successful, you can claim back the exact costs of your legal fees (up to a maximum of £1,500) and the court cost of obtaining a grant of probate or confirmation in support of your application. This means:
- The exact cost of your legal fees towards the cost of applying for a grant of probate or confirmation up to a maximum of £1500 and/or;
- The court fee for obtaining a grant of probate, confirmation or letters of administration (exact amount).
Expenses do not include costs associated with searching probate/confirmation records.
After completing this form
You should send your compensation application form and certified colour copies of your supporting documentation to the Infected Blood Support Scheme in the part of the UK where the deceased person was infected.
