   Added translations in Arabic, Bangla, Chinese, Traditional Chinese, French, Gujarati, Polish, Portuguese, Punjabi Gurmukhi, Spanish, Urdu and an easy read version of the leaflet in English.

   Updated to reflect changes to hospital discharge guidance following the end of the national discharge fund.
