Details

This prison service instruction (PSI) explains the responsibilities of prisons to report, record and investigate accidents, injuries and illnesses among prison staff, prisoners and third parties. This includes:

  • accidents
  • fires
  • work-related ill health
  • control and restraint injuries
  • assaults
  • near misses

It outlines the processes and actions necessary to ensure incidents are reported and investigated quickly and effectively and help reduce the likelihood and impact of such incidents in the future.

It also covers:

  • the investigation process
  • internal reports
  • criteria for reporting events externally
  • the sharing of data between internal systems

This is a current PSI. It came into effect on 16 June 2016.
