OV09: Payment of Official Veterinarian (OV) fees

Use the OV09 form to tell APHA that you have read the OV10 guidance on self billing and will accept self-billing invoices for OV work from Defra.

You must read the OV10 guidance on self billing before completing form OV09.

You will be asked to give:

• your name and position
• practice details, including VAT registration number if relevant

Send the completed form to the Head of Field Delivery for the country your practice is in. For example:

• in England, the Head of Field Delivery England (HFDE)
• in Scotland, the Head of Field Delivery Scotland (HFDS), or
• in Wales, the Head of Field Delivery Wales (HFDW)
