OV45: Application for an official stamp replacement

If you are an OV, CSO or FCCO, you must use this form to request a new stamp if yours has been lost, damaged or stolen.

Details

This form is for use by Official Veterinarians (OVs), Certification Support Officers (CSOs) and Food Competent Certifying Officers (FFCOs).

You’ll be asked to provide:

• your name
• your SP, CSO or NV number
• your practice details
• your crime reference number, if you have one

Applications will be processed in 10 working days. For OVs, replacement stamps will be posted to the address given on your Improve International account. For AHPs and FCCOs, replacement stamps will be posted to your employer’s address.
