Details

You’ll be asked to provide:

  • your OV name
  • your SP number
  • your practice contact information
  • the date from which your OV status will no longer be required

Send your completed form to APHA’s OV team at CSCOneHealthOVTeam@apha.gov.uk, or

When your authorisation has ended you must destroy your official OV stamp.
