Details

Sam is the Animal and Plant Health Agency’s online system for submitting tuberculosis (TB) test results online.

To change your name on Sam, you’ll be asked to provide:

• your current OV or ATT name
• your SP or ATT number
• your practice contact information
• the new name you want to use on Sam

The details you provide must match those currently held on your Improve International training record. You must update your personal information there before submitting this form.
