Use this form to tell the Animal and Plant Health Agency (APHA):

• that you’ve moved to a new practice, if you’re an OV or ATT
• about a change to a practice’s details, such as name or address

Once your form has been processed, APHA’s OV team will update all relevant details on Sam, APHA’s online system for recording tuberculosis (TB) test results.

APHA uses these details to send you important information about your OV authorisation. It’s your responsibility to make sure the details are correct.
