Poultry meat model health certificates: version history

Applies to England, Scotland and Wales

Competent authorities must provide a health certificate to exporters of animals and animal products to Great Britain (England, Scotland and Wales), the Channel Islands and the Isle of Man.

Competent authorities should use the updates below to make changes to health certificates for exporters to ensure they are in line with the current version of the model health certificates.

GBHC330 POU Poultry meat

Version 1.3

Published July 2024:

  • Part I Box I.28: ‘Approval number of cutting plant’ amended to ‘Approval number of cutting and/or Minced Meat Plant’. 
  • Attestation AW/001: renamed to AW/001A. 
  • ()[meat] ()[minced meat] has been added to the opening paragraph of the public health section of Part II of the certificate 
  • Attestation PH/I001: Has been replaced by PH/I001A which has deleted ‘section 2 and section 3 of’ and added ‘and Regulation (EU) No 2019/624’ to the notes for completion. 
  • Attestation PH/MK001: renamed PH/MK001A 
  • Attestation PH/MB001A: is replaced by PH/MB001B 
  • Attestation PH/S101A: added. 
  • Part III notes for completion Part I: Definition of ‘Minced meat’ and foot notes on box reference I.28 have been added.  
  • ‘Retained’ has been changed to ‘Assimilated’ throughout the certificate.

Published April 2024 accepted until January 2025:

  • Attestation AH/T105 added “has been obtained from poultry which has been kept in”
